一、招標(biāo)條件

本密山市人民醫(yī)院進(jìn)口醫(yī)療設(shè)備采購(gòu)項(xiàng)目（招標(biāo) (1) For foreign and overseas bidders, relevant documents registered in the place where they operate according to the laws of that country (region);
（2）對(duì)于國(guó)內(nèi)投標(biāo)人，若為生產(chǎn)企業(yè)，必須具有所投產(chǎn)品醫(yī)療器械生產(chǎn)許可證或第一類醫(yī)療器械生產(chǎn)備案憑證（國(guó)外制造商除外）；若為代理企業(yè)，必須具有第二類、三類醫(yī)療器械經(jīng)營(yíng)許可證或第二類醫(yī)療器械經(jīng)營(yíng)備案憑證，并提供所投產(chǎn)品醫(yī)療器械生產(chǎn)許可證或第一類醫(yī)療器械生產(chǎn)備案憑證（國(guó)外制造商除外）。投標(biāo)產(chǎn)品必須具有醫(yī)療器械注冊(cè)證或第一類醫(yī)療器械備案憑證。
（2)For the domestic bidder, if it is a manufacturer, it must hA/Ve the medical device production license or the record certificate of class I medical device production (excluding foreign manufacturers);If it is an agent enterprise, it must hA/Ve business license of Class II or Class III medical devices or business record certificate of Class II medical devices, and provide production license of medical devices of the invested products or production record certificate of Class I medical devices (except foreign manufacturers).The bid products must hA/Ve registration certificate of medical device or filing certificate of Category I medical device.
（3）擬參加本項(xiàng)目的投標(biāo)人應(yīng)具備《中華人民共和國(guó)政府采購(gòu)法》第二十二條規(guī)定的資格條件：
(3) The bidder who intends to participate in the project shall meet the qualifications stipulated in Article 22 of the Government Procurement Law of the People's Republic of China:
1.具有獨(dú)立承擔(dān)民事責(zé)任的能力；
1. Ability to bear civil liability independently;
2.具有良好的商業(yè)信譽(yù)和健全的財(cái)務(wù)會(huì)計(jì)制度；
2. HA/Ving good commercial reputation and sound financial accounting system;
3.具有履行合同所必需的設(shè)備和專業(yè)技術(shù)能力；
3. HA/Ve the necessary equipment and professional and technical ability to perform the contract;
4.有依法繳納稅收和社會(huì)保障資金的良好記錄；
4. HA/Ve a good record of paying taxes and collecting social security funds according to law;
5.參加政府采購(gòu)活動(dòng)前三年內(nèi)，在經(jīng)營(yíng)活動(dòng)中沒(méi)有重大違法記錄；
5. Within three years before participating in government procurement activities, there were no major illegal records in business activities.
6.法律、行政法規(guī)規(guī)定的其他條件；
6. Other conditions prescribed by laws and administrative regulations;
（4）擬參加本項(xiàng)目的投標(biāo)人須在黑龍江省 上注冊(cè)登記并備案合格；
(4) Bidders who intend to participate in this project must register and file qualified online in government procurement of Heilongjiang Province;
（5）擬參加本項(xiàng)目的投標(biāo)人所投進(jìn)口產(chǎn)品須具有合法來(lái)源證明文件；
(5) The imported products invested by the bidders who intend to participate in the project must hA/Ve a certificate of legal origin;
（6）參加政府采購(gòu)的供應(yīng)商應(yīng)提供參加政府采購(gòu)活動(dòng)近3年內(nèi)在經(jīng)營(yíng)中無(wú)重大違法記錄的聲明或承諾；
(6) Providers participating in government procurement shall provide declarations or promises that they hA/Ve no major illegal records in their operations during the past three years of participating in government procurement activities;
（7）擬參加本項(xiàng)目的投標(biāo)人、法定代表人及授權(quán)委托人均未被司法機(jī)關(guān)列入失信被執(zhí)行人名單，以中華人民共和國(guó)最高人民法院官方網(wǎng)站（shixin.court.gov.cn/index.html）查詢結(jié)果為準(zhǔn)，被列入上述名單的投標(biāo)人不得參加本項(xiàng)目；
(7) The bidders, legal representatives and authorized trustees who intend to participate in the project hA/Ve not been listed by the judicial organs as the Executees of dishonesty. The results of the inquiry on the official website of the Supreme People's Court of the People's Republic of China (shixin.court.gov.cn/index.html) shall prevail. Bidders who are listed in the above-mentioned list shall not be allowed to participate in the project. ;
（8）擬參加本項(xiàng)目的投標(biāo)人未被列入企業(yè)經(jīng)營(yíng)異常名錄、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信名單，以信用中國(guó)官方網(wǎng)站 查詢結(jié)果為準(zhǔn)，被列入上述名單的投標(biāo)人不得參加本項(xiàng)目；
(8) Bidders who intend to participate in this project are not included in the list of abnormal business operations of enterprises, the list of parties involved in major tax violations, and the list of serious violations and dishonesty in government procurement. The results of the inquiry on the official website of China ( ) shall prevail. Bidders who are included in the above list shall not be allowed to participate in this project.
（9）與招標(biāo)人存在利害關(guān)系可能影響采購(gòu)公正性的法人、其他組織或者個(gè)人，不得參加投標(biāo)；單位負(fù)責(zé)人為同一人或者存在控股、管理關(guān)系的不同單位，不得同時(shí)參加同一標(biāo)包投標(biāo)或者未劃分標(biāo)包的同一項(xiàng)目投標(biāo)；集團(tuán)公司中母公司與子公司不得同時(shí)參加同一標(biāo)包投標(biāo)或者未劃分標(biāo)包的同一項(xiàng)目投標(biāo)；同一公司具有獨(dú)立法人的子公司同時(shí)參加同一標(biāo)包投標(biāo)或者未劃分標(biāo)包的同一項(xiàng)目投標(biāo)時(shí)最多不得超過(guò)兩家（以登記的先后順序?yàn)闇?zhǔn)）；
(9) Legal persons, other organizations or individuals with an interest in the tenderer who may affect the fairness of the procurement shall not participate in the bidding; the person in charge of the unit shall be the same person or the different units with a controlling or management relationship, and shall not participate in the same tender or the same project tender without dividing the tenders at the same time; and the group company shall not participate in the same tender or the same project tender without dividing the tenders. The parent company and subsidiary company shall not participate in the same tender or the same project tender without dividing the tender at the same time; the subsidiary company with independent legal person of the same company shall not participate in the same tender or the same project tender without dividing the tender at most two (according to the order of registration);
（10）未領(lǐng)取招標(biāo)文件不可以參加投標(biāo)。
（10）No bidding is allowed without the tender documents.
（11）本項(xiàng)目不接受聯(lián)合體投標(biāo)。
（11）Joint bids for this project will not be accepted.

本項(xiàng)目不允許聯(lián)合體投標(biāo)。
